Alzheimer’s Disease: New Survey and Research Study on Awareness, Testing and Prevention

Very inter­est­ing new data rein­forc­ing two main themes we have been ana­lyz­ing for a while:
1) We bet­ter start pay­ing seri­ous atten­tion (and R&D dol­lars) to lifestyle-based and non-inva­sive cog­ni­tive and emo­tion­al health inter­ven­tions, which are most­ly ignored in favor of inva­sive, drug-based options
2) Inter­ven­tions will need to be per­son­al­ized. The study below ana­lyzes data at the coun­try lev­el, but the same log­ic applies to the indi­vid­ual level

Alzheimer’s Early and Accurate Diagnosis: Normal Aging vs. Alzheimer’s Disease

(Edi­tor’s Note: I recent­ly came across an excel­lent book and resource, The Alzheimer’s Alzheimer's Disease Action PlanAction Plan: The Experts’ Guide to the Best Diag­no­sis and Treat­ment for Mem­o­ry Prob­lems, just released in paper­back. Dr. Murali Doraiswamy, one of the authors and lead­ing Alzheimer’s expert, kind­ly helped us cre­ate a 2‑part arti­cle series to share with Sharp­Brains read­ers advice on a very impor­tant ques­tion, “How can we help the pub­lic at large to dis­tin­guish Alzheimer’s Dis­ease from nor­mal aging — so that an inter­est in ear­ly iden­ti­fi­ca­tion does­n’t trans­late into unneed­ed wor­ries?” What fol­lows is an excerpt from the book, pages 3–8).

Jane, fifty-sev­en, man­aged a large sales force. She prid­ed her­self on being good at names, and intro­duc­tions were easy for her—until last spring when she referred to Bar­bara as Bet­ty at a meet­ing and had to cor­rect her­self. She start­ed notic­ing that her mem­o­ry wasn’t as depend­able as it once was—she had to real­ly try to remem­ber names and dates. Her moth­er had devel­oped Alzheimer’s in her late sev­en­ties, so Jane enter­tained a wide array of wor­ries: Is this just aging? Is it because of menopause? Is it ear­ly Alzheimer’s? Did her cowork­ers or fam­i­ly notice her slips? Should she ask them? Should she see a doc­tor, and if so, which doc­tor? Would she real­ly want to know if she was get­ting Alzheimer’s? Would she lose her job, health insur­ance, or friends if she did have Alzheimer’s?

As it turns out, Jane did not have Alzheimer’s. She con­sult­ed a doc­tor, who, in doc­s­peak, told her that the pas­sage of time (get­ting old­er) had tak­en a slight toll on her once-superquick mem­o­ry. She was slow­ing down a lit­tle, and if she relaxed, the name or date or oth­er bit of infor­ma­tion she need­ed would come to her soon enough. She was still good at her job and home life. She had sim­ply joined the ranks of the wor­ried well.

Nor­mal brain aging, begin­ning as ear­ly as the for­ties in some peo­ple, may include:

  • Tak­ing longer to learn or remem­ber information
  • Hav­ing dif­fi­cul­ty pay­ing atten­tion or con­cen­trat­ing in the midst of distractions
  • For­get­ting such basics as an anniver­sary or the names of friends
  • Need­ing more reminders or mem­o­ry cues, such as promi­nent appoint­ment cal­en­dars, reminder notes, a phone with a well­stocked speed dial

Although they may need some assis­tance, old­er peo­ple with­out a men­tal dis­or­der retain their abil­i­ty to do their errands, han­dle mon­ey, find their way to famil­iar areas, and behave appropriately.

Novem­ber-Decem­ber News: No month goes by with­out sig­nif­i­cant news in the field of cog­ni­tive fit­ness. Sum­ma­rized here are 10 recent devel­op­ments wor­thy of atten­tion, includ­ing an upcom­ing brain train­ing prod­uct for ice hock­ey play­ers, my lec­ture at New York Pub­lic Library, and more.

Inter­views: Videogames, Meditation

Are videogames good for your brain?: A land­mark study by Dr. Arthur Kramer and col­leagues has shown that play­ing a strat­e­gy videogame can bring a vari­ety of sig­nif­i­cant men­tal ben­e­fits to old­er brains. Anoth­er recent study, also by Kramer and col­leagues, does not show sim­i­lar ben­e­fits to younger brains (despite play­ing the same game). How can this be? Dr. Kramer, who has kind­ly agreed to serve on Sharp­Brains’ Sci­en­tif­ic Advi­so­ry Board, elaborates.

Med­i­ta­tion on the Brain: Dr. Andrew New­berg pro­vides an excel­lent overview of the brain ben­e­fits of prac­tices such as med­i­ta­tion. He rec­om­mends, “look for some­thing sim­ple, easy to try first, ensur­ing the prac­tice is com­pat­i­ble with one’s beliefs and goals. You need to match prac­tice with need: under­stand the spe­cif­ic goals you have in mind, your sched­ule and lifestyle, and find some­thing practical.” 

The Need for Objec­tive Assessments 

Cog­ni­tive screen­ings and Alzheimer’s Dis­ease: The Alzheimer’s Foun­da­tion of Amer­i­ca just released a thought­ful report advo­cat­ing for wide­spread cog­ni­tive screen­ings after the age of 65 (55 giv­en the right con­di­tions). Sharp­Brains read­ers, probed by Dr. Joshua Stein­er­man, seem to agree.

Quan­ti­ta­tive EEG for ADHD diag­no­sis: Dr. David Rabin­er reports on the find­ings from a recent study that doc­u­ments the util­i­ty of Quan­ti­ta­tive EEG as an objec­tive test to assist in the diag­no­sis of ADHD. If this pro­ce­dure were to become more wide­ly used, he sug­gests, the num­ber of chil­dren and ado­les­cents who are inap­pro­pri­ate­ly diag­nosed and treat­ed for the dis­or­der would dimin­ish substantially.

Shall we ques­tion the brand new book of human trou­bles?: The fights over the new ver­sion of the psy­chi­atric diag­nos­tic man­u­al, the DSM‑V, are start­ing to come to light. Dr. Vaugh­an Bell won­ders why the pub­lic debate avoids the key ques­tion of whether diag­no­sis itself is use­ful for men­tal health and why psy­cho­met­rics are sim­ply ignored.

Resources for Life­long Learning

Edu­ca­tion builds Cog­ni­tive Reserve for Alzheimers Dis­ease Pro­tec­tion: Dr. Pas­cale Mich­e­lon reviews a recent study that sup­ports the Cog­ni­tive Reserve hypoth­e­sis — men­tal­ly stim­u­lat­ing expe­ri­ences through­out life, such as for­mal edu­ca­tion, help build a reserve in our brains that con­tributes to a low­er prob­a­bil­i­ty of devel­op­ing Alzheimer’s symptoms.

5 Tips on Life­long Learn­ing & the Adult Brain: Lau­rie Bar­tels asks us to please please 1) chal­lenge our­selves with new learn­ing, 2) remem­ber that neu­ro­plas­tic­i­ty and neu­ro­ge­n­e­sis are hall­marks of our brains, 3) check for mis-learn­ing on an ongo­ing basis, 4) more visu­als, less text, 5) move it, move it — start today!

Neu­ro­science Core Con­cepts: We all have heard “Use It or Lose It”. Now, what is “It”? The Soci­ety for Neu­ro­science (SfN) has just released a user-friend­ly pub­li­ca­tion titled Neu­ro­science Core Con­cepts, aimed at help­ing edu­ca­tors and the gen­er­al pub­lic learn more about the brain.

Brain Health: Physical or Mental Exercise?

Our fel­low blog­ger Jere­my over at Psy­Blog has writ­ten a thought­ful post com­par­ing the val­ue of a num­ber of cog­ni­tive enhanc­ing tools. His over­all verdict?

“The evi­dence for exer­cise boost­ing cog­ni­tive func­tion is head-and-shoul­ders above that for brain train­ing, drugs, nutri­tion­al sup­ple­ments and med­i­ta­tion. Sci­en­tif­i­cal­ly, on the cur­rent evi­dence, exer­cise is the best way to enhance your cog­ni­tive func­tion. And as for its side-effects: yes there is the chance of an injury but exer­cise can also reduce weight, low­er the chance of demen­tia, improve mood and lead to a longer life-span. Damn those side-effects!”

Arti­cle: Which Cog­ni­tive Enhancers Real­ly Work: Brain Train­ing, Drugs, Vit­a­mins, Med­i­ta­tion or Exercise?

Jere­my, I start­ed writ­ing this as a com­ment to your post in your blog, but then it got too long. Let me write my reac­tion to your post here.

While I appre­ci­ate your analy­sis and share most of your points, I think the “rank­ing” effort (this type of inter­ven­tion is bet­ter than that one) is ulti­mate­ly mis­lead­ing.  It is Rubik's Cube brain exercisebased on a faulty search for a gen­er­al solution/ mag­ic pill for every­one and everything.

If only things were so sim­ple. Per­haps one day there will be research to sup­port that view, but cer­tain­ly not today. A num­ber of inter­ven­tions have shown their val­ue. In dif­fer­ent pop­u­la­tions, and con­texts. For “exer­cise is the best way to enhance your cog­ni­tive func­tion” to be true, one needs to have a pret­ty spe­cif­ic under­stand­ing of “best”, “your” and “cog­ni­tive function”.

Brain Training Games: Context, Trends, Questions

A spate of recent news cov­er­age on brain fit­ness and “brain train­ing” reflects a grow­ing inter­est in nat­ur­al, non-drug-based inter­ven­tions to keep our brains sharp as we age. This inter­est is very time­ly, giv­en the aging pop­u­la­tion, increas­ing Alzheimer’s rates, and soar­ing health care costs that place more empha­sis than ever on pre­ven­tion and chang­ing lifestyle.

This past Tues­day, the MIT Club of North­ern Cal­i­for­nia, the Amer­i­can Soci­ety on Aging, and Smart­Sil­vers spon­sored an event on The Emerg­ing Brain Fit­ness Soft­ware Mar­ket: Build­ing Bet­ter Brains to explore the real­i­ties and myths of this grow­ing field. The pan­el was mod­er­at­ed by Zack Lynch, Exec­u­tive Direc­tor of the Neu­rotech­nol­o­gy Indus­try Orga­ni­za­tion, and com­posed of a ven­ture cap­i­tal­ist and 3 CEOs of pro­gram devel­op­ers in the field. Before the pan­el, I had the chance to present an overview of the state of the Brain Fit­ness Soft­ware Mar­ket based on our upcom­ing report to be released on March 4th.

Why are we talk­ing about this field at all? Well, for one, an increas­ing num­ber of com­pa­nies are achiev­ing sig­nif­i­cant com­mer­cial suc­cess in pack­ag­ing “brain exer­cise”. An exam­ple is the line of Nin­ten­do games, such as Brain Age and Brain Train­ing, that have shipped over 15 mil­lion units world­wide despite lim­it­ed sci­en­tif­ic sup­port, since 2005. Cognitive Reserve and Intellectually Demanding Jobs

Via Med­Jour­nal­Watch we just found this inter­est­ing paper,

Asso­ci­a­tions of job demands and intel­li­gence with cog­ni­tive per­for­mance among men in late life. Guy G. Pot­ter PhD*, Michael J. Helms BS, and Bren­da L. Plass­man PhD Neu­rol­o­gy 2007.

- CONCLUSIONS: “Intel­lec­tu­al­ly demand­ing work was asso­ci­at­ed with greater ben­e­fit to cog­ni­tive per­for­mance in lat­er life inde­pen­dent of relat­ed fac­tors like edu­ca­tion and intel­li­gence. The fact that indi­vid­u­als with low­er intel­lec­tu­al apti­tude demon­strat­ed a stronger pos­i­tive asso­ci­a­tion between work and high­er cog­ni­tive per­for­mance dur­ing retire­ment sug­gests that behav­ior may enhance intel­lec­tu­al reserve, per­haps even years after peak intel­lec­tu­al activity.”

This is con­sis­tent with the Cog­ni­tive Reserve the­o­ry we dis­cussed in the inter­view with neu­ro­sci­en­tist Yaakov Stern:

- AF (Alvaro Fer­nan­dez): OK, so our goal is to build that Reserve of neu­rons, synaps­es, and skills. How can we do that? What defines “men­tal­ly stim­u­lat­ing activ­i­ties” or good “brain exercise”?

- YS (Yaakov Stern): In sum­ma­ry, we could say that “stim­u­la­tion” con­sists of engag­ing in activ­i­ties. In our research almost all activ­i­ties are seen to con­tribute to reserve. Some have chal­leng­ing lev­els of cog­ni­tive com­plex­i­ty, and some have inter­per­son­al or phys­i­cal demands. In ani­mal stud­ies, expo­sure to an enriched envi­ron­ment or increased phys­i­cal activ­i­ty result in increased neu­ro­ge­n­e­sis (the cre­ation of new neu­rons). You can get that stim­u­la­tion through edu­ca­tion and/ or your occu­pa­tion. There is clear research show­ing how those two ele­ments reduce the risk. Now, what is very excit­ing is that, no mat­ter one’s age, edu­ca­tion and occu­pa­tion, our lev­el of par­tic­i­pa­tion in leisure activ­i­ties has a sig­nif­i­cant and cumu­la­tive effect. A key mes­sage here is that dif­fer­ent activ­i­ties have inde­pen­dent, syn­er­gis­tic, con­tri­bu­tions, which means the more things you do and the ear­li­er you start, the bet­ter. But you are nev­er stuck: bet­ter late than never.

In short, men­tal­ly and social­ly stim­u­lat­ing activ­i­ties, through our edu­ca­tion, occu­pa­tion AND leisure activ­i­ties, con­tribute to build­ing a Cog­ni­tive Reserve in our brains that may help delay mem­o­ry prob­lems, Mild Cog­ni­tive Impair­ment, and Alzheimer’s relat­ed symp­toms, and help main­tain cog­ni­tive per­for­mance over­all as we age.
